Year 6 School Nurse Health Questionnaire - Mar 1, 2021
As part of The National Healthy Child Programme, The Gateshead 0-19 Growing Healthy Team (school nurses) offer parents/carers an opportunity to complete a health questionnaire for all children in year 6.
As a result of Covid 19 restrictions currently in place, the safest way to complete this questionnaire is by telephone.
The Gateshead School Health Screening Team will contact parent/ carers of year 6 children over the coming weeks and months by telephone.
The health information gathered enables the school nurse team to provide the appropriate public health support required to promote children’s health and wellbeing. This also gives families the opportunity to discuss any concerns or health issues with a member of the school nursing team.
